Forms & Materials

All participants must complete and return the following by 5/5/10:
  • Registration & Food Preference Form (accompanied by $75 deposit or full camp fee)
  • Medical Consent Form
  • New Medical Form (may be turned in late if scheduling is an issue)
  • BSA Application (youth if under 21 at time of camp OR adult if over 21 at time of camp)
    • If you are a member of another ESAR post, you may substitute a copy of proof of membership for this document
IF you are under 18, you MUST include a Parental Consent Form!!!
