Cal-OES Disaster Service Worker (DSW) Swear-In

Swear in will be immediately after dinner on Tech Skills weekend at Point Bonita YMCA

Prerequisites

Completion of at least the following:
Fill out your DSW form (MS word version, PDF version), sign it, date it, scan it, and email it to records@calesar.org. 
Notes 
  • Cal-ESAR members under 18 must also have a parent/guardian's signature
  • ESAR ID photos will be added by the unit. If you missed getting an ID photo made, have a photo taken, in uniform,  suitable for a US passport and email an electronic copy of the photo to records@calesar.org.
