New Member Application and Info


Overview

Cal-ESAR operates at the direction for California Office of Emergency Services (OES), Law Enforcement division. Consequently we need all members to complete several forms required by OES. Cal-ESAR has had a youth component since it's inception in 1969 as a Explorer post. Currently the unit includes a BSA Crew and we require our youth members to be part of BSA and that Crew. Youth applications need to include an application to BSA.

All the following forms are required before you begin training. A complete application includes:

  3. Background check
  4. Disaster Service worker (DSW)
  5. Medical Consent
  6. Medical exam
  7. Payment (only after you have been accepted into the class!)

Cal-ESAR Application Forms

Adult Application (18 or older)

Please read and follow the directions below carefully...it saves everyone a bunch of time.


Step #1
  • Application Info Use this link to complete the basic application. All candidates need to complete this, earlier is better! 
    • (Anyone with a felony conviction will not pass the background check. Please do not apply)
  • Selfie- Please take a quick selfie, name it: Last Name_First Name and send to 'records@cal-esar.org' when you submit your application. (Don't worry this is only for your application. Your unit Id photo will be done at Orientation)
  • Save this date: Saturday, September, 29th.  10:30- mid afternoon- New member Orientation and all unit BBQ.


Step # 2

Once you have been notified of acceptance into the candidate class you will need do to complete the following documents within 1 week of notification.

(even if you're done before for some other organization or reason). The following documents must be scanned as a PDF. (no cell phone pics)
    1. Live Scan background check- Download, fill out the 'Applicant Information' section on screen (it's a fillable PDF), 'Save As'.. 'Last name_ First Name_Live' in PDF format. (If you can't fill out on screen, print to paper and complete)
    2. At Will Statement- Download and fill in the At Will Statement electronically it's Word doc). 'Save As'.. 'Last name_ First Name_At Will' in PDF format before printing and signing.
    3. Volunteer Processing form- Download, fill in electronically. Scan and 'Save As' 'Save As'.. 'Last name_ First Name_Vol' in PDF format before printing and signing.
4. Disaster Service Worker (Fill out all the fields, date of birth, phone, etc) on screen, "Save As" using file name format: 'Last Name_First Name_DSW' (e.g. Doe_John_DSW) and send to records@cal-ESAR.org  
    • Option B- Download, print to paper and fill out fields, sign / scan and Save using file name format above and send to records@cal-ESAR.org.
5. Medical Consent (Download, fill out fields, print, sign, scan, Save As 'Last Name_First Name_consent' and send to records@cal-esar.org.

You should be sending at least *five* documents as a single attachment to 'records@calesar.org with your 'Last name,_First name,_App paperwork' in the subject line.
Bring signed, paper copies of all completed docs to the Orientation in September where you will be finger printed as part of the background check.

Medical Exam form This is not required for the first training weekend, but is due within 30 days of your application. This form (we use a BSA form here) must be filled out by your physician, so schedule your appointment soon. Submit electronically as PDF with file name 'Last Name_First Name_med.PDF" to records@cal-ESAR.org.

Youth application (17 and younger)

  • Boy Scouts of America Youth Application
    1. If this form opens in your browser... download to your computer, open and complete using  Acrobat Reader (download if needed) by clicking on the file name.
    2. Print this form, fill out page 2 and sign.
    3. Scan page 2 and "Save As" a PDF.  ( If you printer isn't also a scanner you can use your cellphone to create a legible photograph)
    4. Name the PDF (or photo) with file name format:  " last name.first name.youth" (e.g. doe.john.youth).
  • Parental Consent (Download,  fill out fields, print and sign)
  • Disaster Service Worker (Fill out on screen, "Save As" using file name format: 'Last Name_First Name_DSW' (e.g. Doe_John_DSW) and send to records@cal-ESAR.org  
    • Option B- Download, print to paper and fill out fields, sign / scan and Save using file name format above and send to records@cal-ESAR.org.
  • Medical Consent (Download, fill out fields, print and sign)

Note to *current* members of a BSA troop - Paid members of a Boy Scout troop, please write " Registered member - XXXX Council, Troop ###" ID # - XXXXXX" at the top of your BSA form.

Payment

The training fee for 2018-19 is $275, $50 of which is your uniform pants. Please do not send until you are notified that you have been accepted into the training class.
  1. The uniform pants are great all around outdoor pants, they're required for training weekends and are part of our unit uniform as a Disaster Service Worker (DSW) with OES.
  2. $225 covers all costs for seven training weekends: Outdoor skills, Intro Nav, SAR skills, Tech skills, Advanced Nav, Skills Eval, Yosemite PSAR
  3. First Aid instruction is not included in training fee. If you already have a current Wilderness First Aid/CPR cert, or better, you do not need to attend this class.

After your receive notification that you have a spot in the training class...
