New Member Application and Info


Overview

Cal-ESAR supports the BSA Explorer program and they provide our insurance during training activities, consequently all members need to submit a BSA application which we use to collect basic member info. Our apologies in advance for such a antiquated paper based process, but that is the state of BSA administrative systems. All the following forms are required for a full application:
  1. Member Application
  2. Medical Consent
  3. Medical exam
  4. Disaster Service worker (DSW)
  5. Background check
  6. Complete profile
  7. Selfie photo

  • Please complete all of the forms by closely following the directions below. When complete email the forms to 'records@cal-esar.org'. 
  • After you send the forms to records@cal-esar.org, please send a 1 line email (ex: 'Your Name....paperwork submitted') to email address Training@cal-esar.org.

Cal-ESAR Application Forms

Adult Application (18 or older)

Please read and follow the directions below carefully...it saves a bunch of time and back or forth otherwise.

  • Medical Exam form This is not required for the first training weekend, but is due within 30 days of your application. This form (we use a BSA form here as well) must be filled out by your physician, so schedule your appointment soon.
  • Adult Application -
    This is an editable PDF form: download to your computer, click on the file name to open, complete and Save. You must open in Acrobat Reader (download here if needed) in order to complete on your computer screen.
    • Ignore the directions on page 1 of the PDF when you open it. Do this instead...
      1. Fill out all the boxes on pages 2-3
      2. "Save" the file with your changes
      3. Print pages 2 and 3 
      4. Physically sign in correct spot on both pages
      5. Scan page 2 &  3, "Save As" using file type 'PDF' and name the file with this format: "Your last name. your first name_adult" (e.g. doe.john_adult.pdf)
        • Important- Sections 3, 4, and 5 on the right column of page 3 are required: (ignore 1 & 2)
          • three references, 
          • previous residence
          • etc.
  • Disaster Service Worker (Fill out on screen, "Save As" using file name format: 'Last Name_First Name_DSW' (e.g. Doe_John_DSW) and send to records@cal-ESAR.org  
    • Option B- Download, print to paper and fill out fields, sign / scan and Save using file name format above and send to records@cal-ESAR.org.
  • Live Scan background check- Cal-ESAR operates under the direction for California Office of Emergency Services (OES) Law Enforcement division. As such members over the age of 18 need to under go a background check. (even if you're done before for some other organization or reason) Anyone with a felony conviction will not pass the background check. Please do not apply.
    • Download and print both word documents. You can fill in Form 2 electronically before printing and signing.
    • Sign both forms (2 and 3), Scan and 'Save As' a PDF. Email to 'records@cal-esar.org' They must be scanned as a PDF.
  • Profile InfoUse this link to provide some profile and emergency contact info.
  • Selfie- This is 2017 after all! ...Please take a quick selfie and send to 'records@cal-esar.org' with the paper. Don't worry this is only for your app. Your unit Id photo will be done once you are in the training class.

Youth application (17 and younger)

  • Boy Scouts of America Youth Application
    1. If this form opens in your browser... download to your computer, open and complete using  Acrobat Reader (download if needed) by clicking on the file name.
    2. Print this form, fill out page 2 and sign.
    3. Scan page 2 and "Save As" a PDF.  ( If you printer isn't also a scanner you can use your cellphone to create a legible photograph)
    4. Name the PDF (or photo) with file name format:  " last name.first name.youth" (e.g. doe.john.youth).
  • Parental Consent (Download,  fill out fields, print and sign)
  • Medical Consent (Download, fill out fields, print and sign)

Note to *current* members of a BSA troop - Paid members of a Boy Scout troop, please write " Registered member - XXXX Council, Troop ###" ID # - XXXXXX" at the top of your BSA form.

Submit the forms 

  1. Please complete all of the forms and email them to: 'records@cal-esar.org' Please send a 1 line email (ex: 'Sam Searcher....paperwork submitted') to email address Training@cal-esar.org after you send it to Records.

Payment

The training fee for 2017-18 is $275, $50 of which is your uniform pants. Please do not send until you are notified that you have been accepted into the training class.
  1. The uniform pants are great all around outdoor pants, they're required for training weekends and are part of our unit uniform as a Disaster Service Worker (DSW) with OES.
  2. $225 covers all costs for seven training weekends: Outdoor skills, Intro Nav, SAR skills, Tech skills, Advanced Nav, Skills Eval, Yosemite PSAR
  3. First Aid instruction is not included in training fee. If you already have a current Basic First Aid/CPR cert you do not need to attend this class.
  4. Training fee payment is due October 1st. (Before the start of Outdoor skills training weekend in October)

After your receive notification that you have a spot in the training class...

In the "New Member Name" box below, enter your preferred first name (ex: Bob instead of Robert) and last name. This will be used for your unit email address: ex: 'Firstname.lastname@cal-esar.org'

After we receive your payment you will receive a email with your Cal-ESAR unit email address and additional instructions. Thanks!
