Prerequisites:  Completion of at least one or more of the following

To prepare for your swear-in, please fill out the ...

1. Obtain a current passport-style photo of yourself (a standard head-shoot works).
   This photo appears in the DSW form, so a digital photo that fits in the space in that doc is ideal. Roughly 1.25in wide x 1.50in tall
   
2. Fill out an OES Disaster Service Worker's Form (DSW)
   
3. Turn it in by
1. email it to training@cal-esar.org or
   
2. fax it to (614) 386-7448 or
   
3. print it 2 copies out and bring the forms to the next meeting
   
4. Sign it at the swear-in meeting. Note: If you are a minor, your parent or guardian must sign the form as well
